Immunization Requirements 2007-08

This chart summarizes the vaccine requirements incorporated in Title 25 Health Services, 99.61-97.72 of the Texas Administrative Code.   Students will not be allowed to enroll if his/her immunization records are not presented to the school upon enrollment and if the student’s immunization records are not current.   The immunization record must be an official copy signed/stamped by the physician or clinic. Cypress-Fairbanks ISD requests that the immunizations be presented at the beginning of the semester in which they are required.

The following immunizations are required for all Texas students:

PPCD/SPEECH AND PRE-KINDERGARTEN

  • Four doses of DPT/DTaP or DT if Pertussis is medically contraindicated
  • Three doses of Polio (IPV)
  • One dose of MMR, on or after 1 st birthday
  • Three doses of HIB (a primary series with one dose on or after 1 st birthday) or 1 HIB on or after 15 months
  • Three doses of Hepatitis B
  • One dose of Varicella, on or after 1 st birthday, or parent/ physician statement of chickenpox illness
  • Pneumococcal Vaccine (a primary series, plus a booster if needed according to Texas immunization requirements).
  • Two doses of Hepatitis A vaccine

GRADES KINDERGARTEN THROUGH 12 (K-12)

VACCINE           REQUIRED DOSES

DPT, DTAP, DT, Td  

Five doses of any combination DTaP/DTP unless 4 th dose was given on or after 4 th birthday.

Students 7 years or older:

  • Three doses of any combination DTP/DTaP/DT/TD vaccine (last one after the 4 th birthday)
  • One dose of Td required ten years after last dose of DTP/DTaP/DT.

POLIO (IPV)

Four doses with last dose after the 4th birthday.  Three doses are acceptable if the third dose is after the fourth birthday.

MMR

Two doses of a measles-containing vaccine with the first dose on or after the first birthday; second dose by age 5 or entry into kindergarten.

HEPATITIS B

Three doses are required for grades K-12

VARICELLA

One dose on or after 1 st birthday for grades K-12
Two doses if vaccine given at 13 years or older.
Serologic proof of immunity or documentation of previous illness may substitute for vaccination

**Receipt of the dose up to (and including) 4 days before the birthday will satisfy the school entry immunization requirement.

 

EXEMPTIONS

The law allows (a) physicians to write a statement stating that the vaccine(s) required would be medically harmful or injurious to the health and well-being of the child, and (b) parents/guardians to choose an exemption from immunization requirements for reasons of conscience, including a religious belief.   The law does not allow parents/guardians to elect an exemption simply because of inconvenience (a record is lost or incomplete and it is too much trouble to go to a physician or clinic to correct the problem).

For children needing medical exemptions, an official document signed by the physician should be submitted to the school.
