If a paraprofessional has previous experience in an accredited public or private school, an original service record will be required by the district. Salary credit for experience earned in out-of-state institutions will not be granted until the original service records are received and verified by the appropriate district personnel. You will need to contact your previous district(s) to obtain an official service record. Click on the appropriate file to download the service record request form.
Please have the service record(s) returned to you for verification of accurate information. Sign the top of the form before submitting the originals to the district. We suggest making a copy for your personal records. It may be submitted to:
