Sports physicals for the 2003-2004 school year will be administered in the athletics
area of Langham Creek High School Saturday, May 17, 2003. Physicals for boys
will be conducted from 8:30 a.m. to noon. Physicals for girls will be conducted
from 1 to 3:30 p.m. The cost for this service is $15. Checks should be made
payable to Dr. James P. Fogarty. State-approved physical forms are available
through the athletics department of the school your child presently attends
and at Langham Creek High School. Parents should complete their section of the
physical form prior to the examination. For additional information, please call
