2005-06 Preparticipation Physical Evaluation Form
Athletic physicals for students planning to play sports during the 2005-06 school year will be administered May 19 at Cypress Falls High School, 9811 Huffmeister Rd. All high school and middle school athletes are welcome. Physicals will be given from 5 to 7 p.m. Athletes are requested to wear shorts. Dr. David McChesney will administer the physicals for $15. Checks should be made payable to Dr. David McChesney. For more information, call the Cypress Falls High training room at 281-856-1054.
