2005-06 Preparticipation Physical Evaluation Form
Athletic physicals for the 2005-06 school year will be administered on Saturday, May 21 at Cy-Fair High School, 22602 Hempstead Hwy. Any high school or middle school athletes in CFISD are welcome to attend. Physicals will be given from 9 a.m. to noon and athletes are requested to wear shorts. Team Physicians, Orthopedic Dr. Scot Frost and Cardiologist Thomas DeBauche, along with male and female medical professionals will be administering the physicals.
