2005-06 Preparticipation Physical Evaluation Form
Athletic physicals for students planning to participate in athletics during the 2005-06 school year will be administered on Wednesday May 18, 2005 at Cypress Ridge High School, 7900 N. Eldridge Parkway. All high school and middle school athletes are welcome. Physicals will be given from 4:30 to 7 p.m. Athletes are requested to wear shorts. Dr. Clifford DePrang will administer the physicals for $15 and cash or money order is accepted. Please make the money orders payable to Dr. Clifford DePrang.
