Parent Notification - Health Screenings
In accordance with the Texas Administrative Code (TAC) Chapter 37, Rule §37.25, CFISD will conduct the following state-mandated health screenings. All screenings will be performed by certified screeners following guidance from the Texas Department of State Health Services.
- Vision and Hearing Screening:
- All students aged 4 and older who are enrolled for the first time
- Students in Pre-K, Kindergarten, 1st, 3rd, 5th and 7th grades
- Spinal Screening
- Girls in 5th and 7th grades
- Boys in 8th grade
- Risk Assessment for Type 2 Diabetes (Acanthosis Nigricans)
- Students in 1st, 3rd, 5th, and 7th grades
If you wish to opt out of any required screening, you may do so by submitting signed and dated documentation from a licensed healthcare provider showing the completed screening results. This documentation must be provided to your campus nurse no later than Sept. 1.
De acuerdo con el Capítulo 37, Regla §37.25 del Código Administrativo de Texas (TAC), el CFISD llevará a cabo las siguientes evaluaciones de la salud exigidas por el estado. Todas las evaluaciones serán realizadas por evaluadores certificados siguiendo la guía del Departamento Estatal de Servicios de Salud de Texas.
- Examen de la vista y la audición:
- Estudiantes de 4 años o más inscritos por primera vez
- Estudiantes de prekínder, kínder, 1.°, 3.°, 5.° y 7.° grado
- Examen de columna vertebral
- Niñas de 5.° y 7.° grado
- Varones de 8.° grado
- Evaluación de riesgos para la diabetes tipo 2 (acantosis nigricans)
- Estudiantes de 1.°, 3.°, 5.° y 7.° grado
Si usted desea optar por no participar en cualquiera de las evaluaciones obligatorias, podrá hacerlo enviando documentación firmada y fechada por un proveedor de atención médica autorizado que muestre los resultados de la evaluación completa. Esta documentación deberá entregarse a la enfermera de su escuela a más tardar el 1.° de septiembre.
