The school district has received a written request to administer medication from a health care provider with prescriptive authority and the parent, legal guardian or other person having legal control of the student.
Non-prescription medication shall be in the original, unopened container that is clearly labeled and not expired. Substances such as vitamins, herbal preparations, etc. will not be given during school hours.
Parent provided non-prescription medication from the district-approved list may be given up to 3 consecutive days with no physician signature. District-approved nonprescription medications will not be administered in dosages higher than the label recommends for age, or for indications the medication is not FDA approved for in children and adolescents.
District-approved nonprescription medication list:
- Acetaminophen (e.g. Tylenol)
- Calcium Carbonate (e.g. antacids)
- Diphenhydramine (e.g. Benadryl)
- Ibuprofen (e.g. Advil, Motrin)
- Naproxen (e.g. Aleve)
A physician order or physician signature on the Medication Authorization Form is required for all other non-prescription medications, off-label medications and prescription medication samples and must include the condition for which the medication is given. Changes in medication or dosage require a new Medication Permission Form and physician signature/order.
